- Name*
- DOB
- Gender
- SEN Y/N (please provide details if yes)*
- Medical/dietary requirements*
- Emergency contact details*
- Free School Meals Y/N
- Photographic/Video consent**
If you are unable to provide all of this information, please provide as much information as possible and your referral will still be considered.
